Today’s Softball Open Field is canceled due to rain. Tryouts are scheduled for Friday, July 17 and Monday, July 20 from 9-11 AM.

Below is the health requirements for all ages. 🧡💙 CUSD #99 Health Requirements Students will NOT be able to attend school after October 1st unless ALL required physical exams and immunizations are received. Preschool Students: Physical Exam (Parents please complete the yes/no questions on the back and sign and date) Complete immunization record signed by a doctor: check with your healthcare provider for compliance Birth Certificate Kindergarten Students: Physical Exam (Parents please complete the yes/no questions on the back and sign and date) Complete immunization record signed by a doctor: check with your healthcare provider for compliance Dental Exam (dental safari will be here in January to utilize if not done by then) Eye Exam in Illinois (by an eye doctor) form dated within one year of the first day of school Birth Certificate 2nd Grade Students: Dental Exam on IL form (dental safari will be here in January to utilize if not done by then) Sixth Grade Students: Physical Exam (Sports physical NOT acceptable, IL Certificate of Child Health Exam form required) Parents please complete yes/no questions on the back and sign and date Complete immunization record signed by a doctor, including (1)Tdap,(1)Meningitis-these are 6th grade requirements Dental Exam (dental safari will be here in January to utilize if not done by then) Ninth Grade Students: Physical Exam (Sports physical NOT acceptable, IL Certificate of Child Health Exam form required)-Parents please complete yes/no questions on the back and sign and date. Complete immunization record. Dental Exam (dental safari will be here in January to utilize if not done yet). Twelfth Grade Students: Proof of receiving (2) Meningitis vaccines: 2nd one after 16th birthday. Sports Physicals Required for all athletes annually. 6th and 9th graders do not need a separate sports physical since they should have a regular school physical on file. Turn this into the nurse also. Other Important Information Religious exemption for immunizations requires an Illinois Certificate of Religious Exemption form completed by parents and their family physician. Please fill out the BLUE medical form with any allergies, medication, and medical information that the nurse needs to know. If the student must take prescription drugs during school hours, a written order from their provider must be on file. If you are moving into our district from out of state, all medical forms are required (Illinois physical, immunization record, Illinois eye exam, Illinois dental exam). The CHS boys and girls basketball teams would like to thank all of the young players who attended basketball camp this week! We loved seeing everyone work hard, improve each day, and have fun on the court. The future is bright for Bearcat basketball! 🏀🧡💙

The CES 8th grade graduation is tonight in the Richard E. Towers Gymnasium. The doors open at 5:30 PM, ceremony begins at 6:00 PM.
